ToolsGavel verdict
Fillout has the friendliest seat and entry-price math of the five builders we compared: unlimited collaborators, a 1,000-response free plan with payments, and $15 a month on annual billing for the first paid tier. We hold the rating at 4.1 because only 48 Trustpilot reviews and about 30 on G2 back it up, HIPAA is off the table, and the pricing page leaves monthly-billed prices unstated. Strong for a small team, but not yet proven the way Jotform is.
Best for: Small teams that want unlimited collaborators, payments on the free plan and routing or PDF workflows, and who are comfortable with a vendor that has only 48 Trustpilot reviews
What we liked
- Unlimited seats on every plan, including free, so adding a teammate never changes the price (fillout.com/pricing)
- The free plan is large: 1,000 responses a month with unlimited forms and payments included (fillout.com/pricing)
- Lowest annual entry price of the five builders we compared: Starter is $180 a year ($15/mo) for 2,000 responses a month (fillout.com/pricing)
- Unlimited responses at Business, $900 a year ($75/mo), where response-metered rivals charge more as volume grows (fillout.com/pricing)
- Workflow features beyond basic forms: routing, PDF generation, scheduling, signatures, login-protected editable forms and 50+ native integrations (fillout.com)
What to watch
- Very small review base: Trustpilot shows 3.5 out of 5 from just 48 reviews, and secondary sources show a G2 score from around 30. Treat the ratings as early signals, not a track record (Trustpilot)
- No HIPAA story: the pricing page does not mention it, and a third-party vendor says Fillout will not sign a BAA, which we could not verify. Clinics should look at [Jotform](/tools/jotform/) instead (fillout.com/pricing, Paubox)
- All branding comes off only at Pro, $40/mo billed annually, and Starter keeps 'light branding' (fillout.com/pricing)
- Pricing pages leave gaps: monthly-billed prices for Pro and Business are not shown, and the response definition is not documented. One Trustpilot reviewer called the pricing misleading over viewing extra submissions (fillout.com/pricing, Trustpilot)

Fillout builds an online form and workflow builder with payments, file upload, routing, PDF generation, scheduling and signatures, plus a free plan that allows 1,000 responses a month. Pricing starts at $15 a month billed annually for the first paid plan and tops out at $75 a month annually for unlimited responses. Trustpilot rates it 3.5 out of 5, but from only 48 reviews as of November 7, 2026, so the score is a thin sample. See where it sits in our best online form builders guide.
Fillout’s parent company renamed itself Zite, according to a post on Fillout’s support blog that a news index dates to March 9, 2026. Fillout stays a product at fillout.com, and its homepage header now reads “by Zite.”
What Fillout is for
Fillout targets teams that want a form to do something after submission. The feature list includes Stripe payments, file upload, login-protected forms that respondents can edit, routing and workflows, PDF generation, scheduling, signatures, 50+ native integrations, white-label options, custom domains and QR codes. It leans toward form and workflow use, not survey research, so it has no counterpart to the crosstabs and randomization at SurveyMonkey.
The seat policy is the clearest difference from Jotform. Fillout gives unlimited seats on every plan, free included, while Jotform is single-user below Enterprise. For a three-person team, that alone can decide the purchase.
What you get on each plan
| Plan | Annual (per mo) | Responses/mo | Branding | Seats |
|---|---|---|---|---|
| Free | $0 | 1,000 | Fillout badge | Unlimited |
| Starter | $180/yr ($15/mo) | 2,000 | Light branding | Unlimited |
| Pro | $480/yr ($40/mo) | 5,000 | All branding removed | Unlimited |
| Business | $900/yr ($75/mo) | Unlimited | None | Unlimited |
| Enterprise | Custom | 1,000,000+ records | n/a | Unlimited |
Payments work on every plan, free included. Fillout’s pricing page shows only annual-billed figures. We inferred about $19 a month for Starter from a struck-through $228 a year, and we did not find monthly-billed prices for Pro or Business, so those are left out here. The comparison table on the page calls Business “Standard,” a small naming mismatch we noticed but could not resolve.
Limits and what is held back
Responses are metered per month and reset on the 1st. Fillout says forms keep collecting after the cap, and you can upgrade or wait. That is friendlier than a hard stop, but a Trustpilot reviewer wrote “Misleading pricing. Go premium to view uncompleted submissions,” so confirm what you can see once you pass the limit. The help page that should define a response returned a 404 when we checked.
The free plan also drops CAPTCHA, premium fields, custom themes, custom domains, redirects and analytics beyond a summary, and files are capped at 20 MB. Removing all branding takes Pro at $40 a month annually.
What users say in reviews
Trustpilot shows 3.5 out of 5 from 48 reviews, with 86% at five stars and 8% at one star. With a sample this small, a few reviews move the score.
G2 shows about 4.6 from roughly 30 reviews per secondary sources, and we found no
Capterra listing. We read only five low-star reviews, so these themes are low confidence: a form blocked by an automated flag, slow support (“takes a full day to reply”), a free feature removed without notice, and the pricing complaint above. One reviewer raised a data exposure concern about update forms connected to Airtable or Notion. That claim has one source and we could not verify it.
Fillout claims SOC 2 Type II compliance and AES-256 encryption. Those are the vendor’s statements, not something we audited.
Who should skip it
Skip Fillout if you handle protected health information. Nothing on its pricing page covers HIPAA, and Jotform sells a BAA at Gold. Skip it if you need survey-grade analysis, where SurveyMonkey goes deeper but carries a 2.6 Trustpilot score over billing. Skip it if you want a long public track record before trusting a vendor with customer data, since 48 reviews is thin and the parent company renamed itself (to Zite) in March 2026. For a free plan with no stated response cap, look at Tally, and for one-question-at-a-time design, look at Typeform.
Pricing note: Fillout's pricing page shows annual-billed figures. Free is $0: 1,000 responses a month, unlimited forms and seats, payments, and a Fillout badge on forms, with 20 MB file uploads and no CAPTCHA, premium fields, custom themes, custom domains, redirects or analytics beyond a summary. Starter is $180/yr ($15/mo) for 2,000 responses a month and light branding. The monthly-billed Starter price is not shown directly, and about $19/mo is our inference from a struck-through $228/yr, so treat it as unconfirmed. Pro is $480/yr ($40/mo) for 5,000 responses and removal of all branding. Business is $900/yr ($75/mo) for unlimited responses, and its monthly-billed price is not shown. Enterprise is custom. Responses are metered per month and reset on the 1st. Fillout says forms keep collecting after you hit the cap and you can upgrade or wait for the next month, but one Trustpilot reviewer says viewing the extra submissions requires a paid plan. Fillout's help page defining what counts as a response returned a 404 when we checked.
Frequently asked questions
How much does Fillout cost?
Free is $0 for 1,000 responses a month. Starter is $180 a year ($15/mo) for 2,000 responses, Pro is $480 a year ($40/mo) for 5,000 responses, and Business is $900 a year ($75/mo) for unlimited responses. Enterprise is custom. These are annual-billed figures. Monthly-billed prices for Pro and Business are not shown on the pricing page, and about $19 a month for Starter is our inference, so confirm at checkout.
Is Fillout free?
Yes. The free plan includes 1,000 responses a month, unlimited forms, unlimited seats and payments, with a Fillout badge on your forms. It caps files at 20 MB and leaves out CAPTCHA, premium fields, custom themes, custom domains, redirects and analytics beyond a summary.
What happens when I go over my Fillout response limit?
Fillout says your forms still collect responses and you can upgrade or wait until the limit resets on the 1st of the month. A Trustpilot reviewer says viewing extra or uncompleted submissions requires a paid plan, and the page does not define what counts as a response, so test this on the free plan before relying on it.
Did Fillout change its name?
The product is still Fillout at fillout.com. Its parent company renamed itself Zite, according to a post on Fillout's support blog that a news index dates to March 9, 2026, and the Fillout homepage header now reads 'by Zite'. We found nothing that changes Fillout's pricing or plans, but the company's direction after the rename is not something we could verify.
Does Fillout offer HIPAA compliance?
We found no HIPAA mention on Fillout's pricing page. A third-party vendor, Paubox, says Fillout will not sign a BAA, which we could not verify directly. Fillout claims SOC 2 Type II and AES-256 encryption on its own site. If you handle protected health information, ask Fillout in writing or use Jotform, which lists a BAA on Gold.
